Looked a little bit more into it. The bar was in the basement with only one egress point, a narrow staircase going up. Shit.

Make a brief note of all exits and take the nearest one. All doors must be unlocked during normal business hours. Most sheep will impulsively try to exit the same main door they entered. Great White violated the unwritten rule for performers by not informing the audience to calmly exit the building in this way. That was the difference between 0 lives lost and 100.
