He was taking off on a dirty piece of land and the main rotors whipped up a chunk of something on the ground that flew into the tail rotor, breaking it and a piece of the tail rotor flies off into the main rotor.

That thing may have been ok to fly if he only knew that you have to take off and land only on a clean pad.»
