Daniel Ortega, a 34-year-old North Las Vegas resident and member of the Las Vegas Athletic Club (LVAC), carried out a shooting at the gym located at 1725 N. Rainbow Boulevard. Armed with an Olympic Arms PCR-223 caliber rifle and multiple ammunition magazines, Ortega fired 24 rounds, killing Edgar Quinonez, a 15-year employee and manager at the gym, and injuring three others, one of whom was in critical condition. The shooting began around 1:33 p.m. after Ortega approached the front desk, briefly exchanged words with an employee, and then opened fire, pursuing the employee deeper into the facility.

Ortega’s rifle malfunctioned during the attack, which Las Vegas Metropolitan Police Department (LVMPD) Assistant Sheriff Jamie Prosser credited with preventing a mass casualty event. As Ortega attempted to exit the gym, three officers—Kaid Urban (29), Skeeter Black (44), and Aaron Javier (37)—engaged him outside, firing six rounds and fatally wounding him. Despite attempts to save his life, Ortega died at University Medical Center. Police found no prior connection between Ortega and Quinonez, and the motive remains unclear, with ongoing investigations as of May 20, 2025. Had Ortega survived, he would have faced charges including murder with a deadly weapon, three counts of attempted murder, and assault with a deadly weapon. The officers involved are on paid leave, per standard procedure
