SAN FRANCISCO -- A video appears to show a train operator falling asleep moments before a July 31 crash involving two empty Muni trains. The San Francisco Municipal Transportation Agency says the incident was initially determined to have been caused by operator fatigue. The video obtained by the group NorCal Bodycam and shared with ABC7 Bay Area appears to show the operator asleep before the train crashes into the rear of another train ahead of it, shattering window glass. The collision occurred on the morning of July 31 near the intersection of 47th Avenue and Wawona Street, north of the San Francisco Zoo. Fire officials said both trains were empty of passengers at the time of the crash.

A 16-year-old driver and a pedestrian suffered serious injuries Friday afternoon following a multi-vehicle crash on East Grand Avenue (Route 209) in Tower City Borough that remains under investigation by Pennsylvania State Police. According to a Pennsylvania State Police public information release, the crash occurred at approximately 2:56 p.m. on Friday, July 31, 2026, at the intersection of East Grand Avenue (SR 209) and South 6th Street. State Police said the teenage driver, a 16-year-old male from Sacramento, Schuylkill County, was traveling southbound on East Grand Avenue in a 2012 BMW 545i. At the same time, a 2011 Subaru Outback was attempting to turn left onto Route 209 from the Mid Penn Bank drive-thru ATM parking lot. Investigators allege the BMW was traveling at a high rate of speed when it swerved into the northbound lane to avoid the turning Subaru. The BMW struck the Subaru before continuing northbound, where it collided with three legally parked vehicles—a 2011 Toyota Tacoma, a 2019 Subaru Impreza, and a 2017 Hyundai Elantra. Police said the BMW then struck a 56-year-old pedestrian from Williamstown who had been walking behind one of the parked vehicles. The teenage driver and the pedestrian both sustained significant injuries and were flown by LifeFlight to Hershey Medical Center. A 75-year-old passenger in the Subaru suffered suspected minor injuries and was transported by Tower City Ambulance to Geisinger St. Luke's Hospital. The Subaru's 52-year-old driver was not injured. Both the BMW and the Subaru were towed from the scene, while the Toyota Tacoma was also removed by a towing service. Emergency responders included Life Team EMS, LifeFlight, Tower City Ambulance, the Muir Fire Department, Pennsylvania State Police Troop L Forensic Services Unit, the Troop L CARS Unit, and Hammer Towing. Pennsylvania State Police said criminal charges are pending as the investigation continues.

Surveillance video captures the violent implosion of a massive tank at a Washington state paper mill, an industrial disaster that killed workers and tore through part of the Nippon Dynawave facility in May. The video shows the white liquor tank at the Longview mill suddenly collapse, unleashing the deadly force of the implosion and revealing the scale of the destruction in its immediate aftermath. The video, obtained via a records request, was recorded from a surveillance camera located in the Station 6 Central Maintenance parking lot. The tank is barely visible at the top of the frame. Seconds into the recording, the tank rupture occurs. The tank can be seen collapsing on one side, followed by a large gaseous cloud. Minutes later, that cloud and a flow of white liquor slowly fill the parking lot. The catastrophe unfolded on the morning of May 26. Eleven people were killed, making it the deadliest industrial accident in Washington state history. The ruptured tank spilled up to 570,000 gallons of white liquor, a strong alkaline liquid made mostly of sodium hydroxide and sodium sulfide used in the papermaking process to dissolve wood chips.

The Mercedes-Benz driver left the scene, police said. The driver and passenger of the Corvette, as well as the injured pedestrian, were hospitalized. Officials said they were in stable condition.

Police said a Corvette was speeding eastbound on Sherman Avenue around 6:30 a.m. when it collided with a Mercedes-Benz near West 204th Street. The Corvette then struck a 34-year-old pedestrian before coming to a stop near a building. The Mercedes continued into a parked, unoccupied moped. The driver of the Mercedes fled the scene and remains wanted by police. The Corvette driver, a passenger, and the pedestrian were all taken to Harlem Hospital in stable condition. Investigators are looking into whether the vehicles were drag racing before the crash. The Corvette driver has not been charged.
